Oxidative stress has been proposed as a significant part of the pathogenesis of fibromyalgia, and the phase angle in bioelectrical impedance analysis has been explored as a potential technique to screen oxidative abnormalities. This study recruited 35 women with fibromyalgia and 35 healthy women, who underwent bioelectrical impedance analysis and maximum isometric handgrip strength tests. Women with fibromyalgia showed lower bilateral handgrip strength (right hand: 16.39 ± 5.87 vs. 27.53 ± 4.09, p < 0.001; left hand: 16.31 ± 5.51 vs. 27.61 ± 4.14, p < 0.001), as well as higher body fat mass (27.14 ± 10.21 vs. 19.94 ± 7.25, p = 0.002), body fat percentage (37.80 ± 8.32 vs. 30.63 ± 7.77, p < 0.001), and visceral fat area (136.76 ± 55.31 vs. 91.65 ± 42.04, p < 0.01) compared with healthy women. There was no statistically significant difference in muscle mass between groups, but women with fibromyalgia showed lower phase angles in all body regions when compared with healthy control women (right arm: 4.42 ± 0.51 vs. 4.97 ± 0.48, p < 0.01; left arm: 4.23 ± 0.48 vs. 4.78 ± 0.50, p < 0.001; trunk: 5.62 ± 0.77 vs. 6.78 ± 0.84, p < 0.001; right leg: 5.28 ± 0.56 vs. 5.81 ± 0.60, p < 0.001; left leg: 5.07 ± 0.51 vs. 5.69 ± 0.58, p < 0.001; whole body: 4.81 ± 0.47 vs. 5.39 ± 0.49, p < 0.001). Moreover, whole-body phase-angle reduction was only predicted by the presence of fibromyalgia (R2 = 0.264; ß = 0.639; F(1,68) = 24.411; p < 0.001). Our study revealed significantly lower phase angle values, lower handgrip strength, and higher fat levels in women with fibromyalgia compared to healthy controls, which are data of clinical relevance when dealing with such patients.

In the last few decades, numerous studies pertaining to research groups worldwide have investigated the effects of oral caffeine intake on fat oxidation at rest, during exercise, and after exercise. However, there is no bibliometric analysis to assess the large volume of scientific output associated with this topic. A bibliometric analysis of this topic may be used by researchers to assess the current scientific interest in the application of caffeine as a nutritional strategy to augment fat oxidation, the journals with more interest in this type of publication, and to draw international collaborations between groups working in the same area. For these reasons, the purpose of this study was to assess the research activity regarding oral caffeine intake and fat oxidation rate in the last few decades by conducting a bibliometric and visual analysis. Relevant publications from 1992 to 2022 were retrieved from the Web of Science (WoS) Core Collection database. Quantitative and qualitative variables were collected, including the number of publications and citations, H-indexes, journals of citation reports, co-authorship, co-citation, and the co-occurrence of keywords. There were 182 total publications, while the number of annual publications is saw-shaped with a modest increase of 11.3% from 2000 to 2009 to 2010 to 2019. The United States was the country with the highest number of publications (24.17% of the total number of articles), followed by the Netherlands (17.03%). According to citation analyses, the average number of citations per document is 130, although there are 21 documents that have received more than 100 citations; the most cited document reached 644 citations. These citation data support the overall relevance of this topic in the fields of nutrition and dietetics and sport sciences that when combined harbored 85.71% of all articles published in the WoS. The most productive author was Westerterp-Plantenga with 16 articles (8.79% of the total number of articles). Nutrients was the journal that published the largest number of articles on this topic (6.59% of the total number of articles). Last, there is a tendency to include keywords such as "performance", "carbohydrate", and "ergogenic aid" in the newer articles, while "obesity", "thermogenic", and "tea" are the keywords more commonly included in older documents. Although research into the role of caffeine on fat oxidation has existed since the 1970s, our analysis suggests that the scientific output associated with this topic has progressively increased since 1992, demonstrating that this is a nutritional research area with a strong foundational base of scientific evidence. Based on the findings of this bibliometric analysis, future investigation may consider focusing on the effects of sex and tolerance to caffeine to widen the assessment of the effectiveness of oral caffeine intake as a nutritional strategy to augment the use of fat as a fuel, as these terms rarely appear in the studies included in this analysis. Additionally, more translational research is necessary as the studies that investigate the effect of oral caffeine intake in ecologically valid contexts (i.e., exercise training programs for individuals with excessive adiposity) are only a minor part of the studies on this topic.

The genetic profile that is needed to identify talents has been studied extensively in recent years. The main objective of this investigation was to approach, for the first time, the study of genetic variants in several polygenic profiles and their role in elite endurance and professional football performance by comparing the allelic and genotypic frequencies to the non-athlete population. In this study, genotypic and allelic frequencies were determined in 452 subjects: 292 professional athletes (160 elite endurance athletes and 132 professional football players) and 160 non-athlete subjects. Genotyping of polymorphisms in liver metabolisers (CYP2D6, GSTM1, GSTP and GSTT), iron metabolism and energy efficiency (HFE, AMPD1 and PGC1a), cardiorespiratory fitness (ACE, NOS3, ADRA2A, ADRB2 and BDKRB2) and muscle injuries (ACE, ACTN3, AMPD1, CKM and MLCK) was performed by Polymerase Chain Reaction-Single Nucleotide Primer Extension (PCR-SNPE). The combination of the polymorphisms for the "optimal" polygenic profile was quantified using the genotype score (GS) and total genotype score (TGS). Statistical differences were found in the genetic distributions between professional athletes and the non-athlete population in liver metabolism, iron metabolism and energy efficiency, and muscle injuries (p<0.001). The binary logistic regression model showed a favourable OR (odds ratio) of being a professional athlete against a non-athlete in liver metabolism (OR: 1.96; 95% CI: 1.28-3.01; p = 0.002), iron metabolism and energy efficiency (OR: 2.21; 95% CI: 1.42-3.43; p < 0.001), and muscle injuries (OR: 2.70; 95% CI: 1.75-4.16; p < 0.001) in the polymorphisms studied. Genetic distribution in professional athletes as regards endurance (professional cyclists and elite runners) and professional football players shows genetic selection in these sports disciplines.

PURPOSE: The understanding of fatigue of the human motor system is important in the fields of ergonomics, sport, rehabilitation and neurology. In order to understand the interactions between fatigue and reaction time, we evaluated the effects of two different fatiguing tasks on reaction time. METHODS: 83 healthy subjects were included in a case-control study with three arms where single and double choice reaction time tasks were performed before and after 2 min fatiguing task (an isometric task, a finger tapping task and at rest). RESULTS: After an isometric task, the right-fatigued hand was slower in the choice component of a double choice reaction time task (calculated as the individual difference between single and double choice reaction times); also, the subjects that felt more fatigued had slower choice reaction time respect to the baseline assessment. Moreover, in relationship to the performance decay after two minutes, finger tapping task produces more intense fatigability perception. CONCLUSIONS: We confirmed that two minutes of isometric or repetitive tasks are enough to produce fatigue. The fatigue perception is more intense for finger tapping tasks in relation to the performance decay. We therefore confirmed that the two fatiguing tasks produced two different kind of fatigue demonstrating that with a very simple protocol it is possible to test subjects or patients to quantify different form of fatigue.

PURPOSE: Scant research exists regarding the effects of playing Boccia as a rehabilitation strategy for people with severe mobility limitations due to neuromuscular and other neurological disorders. This study is aimed at identifying the feasibility and effects of playing Boccia on the upper limb impairments of people with severe mobility limitations due to neuromuscular and other neurological disorders. MATERIALS AND METHODS: Seven people played Boccia three times per week for 20 weeks as part of the rehabilitation process, while other seven kept up with their usual rehabilitation schedule. Attrition, adherence, adverse effects, participation and completion rate were registered to assess feasibility. The effects of the program on grip, pinch strength and upper-limb active range of motion were assessed by means of a dynamometer and a goniometer. RESULTS AND CONCLUSIONS: The program was feasible, although no effects were observed after its completion on variables assessed, except for hand flexion and ulnar deviation active range of motion. In a group of people with severe disability due to neuromuscular and other neurological disorders, playing Boccia as part of a multidisciplinary rehabilitation program was shown to be a feasible therapy. However, practicing this game did not lead to significant improvements in upper limb impairments, except for wrist flexion and ulnar deviation active range of motion.

Background: Multiple sclerosis (MS) is an autoimmune disorder of the CNS in which inflammation, demyelination, and axonal damage of the central nervous system coexist. Fatigue is one of the most disabling symptoms in MS and little is known about the neurophysiological mechanisms involved. Methods: To give more mechanistic insight of fatigue in MS, we studied a cohort of 17 MS patients and a group of 16 age-matched healthy controls. Baseline Fatigue Severity Scales and Fatigue Rating were obtained from both groups to check the level of fatigue and to perform statistical correlations with fatigue-induced neurophysiologic changes. To induce fatigue we used a handgrip task. During the fatiguing task, we evaluated fatigue state (using a dynamometer) and after the task we evaluated the Borg Rating of Perceived Exertion Scale. Transcranial magnetic stimulation and peripheral electric stimulation were used to assess corticospinal tract and peripheral system functions before and after the task. Results: Clinically significant fatigue and central motor conduction time were greater in patients than in controls, while motor cortex excitability was decreased and maximal handgrip strength reduced in patients. Interestingly, fatigue state was positively correlated to perceived fatigue in controls but not in patients. Furthermore, in the presence of similar fatigue state over time, controls showed a significant fatigue-related reduction in motor evoked potential (a putative marker of central fatigue) whereas this effect was not seen in patients. Conclusions: in MS patients the pathogenesis of fatigue seems not driven by the mechanisms directly related to corticospinal function (that characterize fatigue in controls) but seems probably due to other "central abnormalities" upstream to primary motor cortex.

Objetivos: Desarrollar recomendaciones sobre la actividad física y el ejercicio para pacientes con espondiloartritis (EspA) basadas en la opinión de expertos. Métodos: Dos grupos de expertos, uno de fisioterapeutas, rehabilitadores y profesionales de la actividad física y deporte y otro de reumatólogos con interés en EspA, se reunieron para discutir los resultados de grupos focales con pacientes sobre barreras al ejercicio y de una encuesta a reumatólogos sobre ejercicio en EspA. A continuación se redactaron unas recomendaciones preliminares que fueron sometidas a la opinión de los expertos de ambos grupos mediante metodología Delphi a dos rondas. Resultados: Se emitieron 21 recomendaciones que cubren el ejercicio físico, la adaptación al paciente, el modo de dar los mensajes, el manejo del dolor, el tipo de ejercicios indicado y el seguimiento. El grado de acuerdo varía ligeramente entre los grupos de expertos pero en general fue alto. Los ítems discordantes o con poco acuerdo fueron eliminados del consenso. Conclusiones: Se han emitido recomendaciones sobre cuándo y cómo prescribir ejercicio físico y monitorizarlo en pacientes con EspA basadas en la opinión de expertos en espondilitis y en la prescripción de ejercicio. Deberemos confirmar si estas recomendaciones son útiles para la práctica clínica y tienen efecto en los pacientes con EspA atendidos por reumatólogos

La fatiga es definida como un fenómeno relacionado con el tiempo de disminución de la capacidad máxima de generación de fuerza, expresándose, como un deterioro en la contracción máxima voluntaria (CMV). La aparición de la fatiga en el músculo esquelético durante un esfuerzo ha sido de interés para los fisiólogos, especialmente porque la fatiga es un factor limitante, tanto en el rendimiento deportivo como en la realización de cualquier tarea. El objetivo del artículo es analizar la literatura y proporcionar una revisión sistemática sobre la fatiga inducida por tareas de contracción muscular, provocadas por contracciones isométricas o mediante tapping de dedos. Para ello se realizó una investigación basada en la metodología PRISMA (Artículos de informes preferidos para revisiones sistemáticas y metanálisis). Llevando a cabo una búsqueda de artículos en las bases de datos PubMed, Medline, Science Direct y Google Scholar, entre los meses de junio y noviembre de 2017 con fecha posterior al año 2000. De los 315 estudios identificados inicialmente, sólo 12 cumplieron con los criterios de selección establecidos. La variabilidad metodológica de los distintos estudios permite observar como a través de las tareas de tapping (movimientos repetitivos de dedos), o mediante tareas de contracción isométrica se pueden determinar los distintos parámetros de la fatiga que se evalúan en cada estudio, siendo un instrumento muy utilizado para abordar dicho tema. La fatiga muscular se ha estudiado predominantemente cuando es inducida por tareas isométricas, habiendo un mayor número de investigaciones que utilizan este tipo de metodología, ya que, aunque el tapping de dedos sea un procedimiento fiable para evaluar los mecanismos neurofisiológicos subyacentes de la fatiga, apenas se ha explorado


Fatigue has been defined as a phenomenon related to the time of decrease of the maximum capacity of generation of force, expressing itself, generally, as a deterioration in the maximum voluntary contraction (CMV). The appearance of fatigue in skeletal muscle during an effort has long been of interest to physiologists, especially because fatigue is a limiting factor in athletic performance or in the performance of any task. The aim of the article is to analyze the literature and provide a systematic review on fatigue induced by muscle contraction tasks, caused by isometric contractions or by finger tapping. To this end, an investigation was carried out based on the PRISMA methodology (Articles of preferred reports for systematic reviews and meta-analyzes). Conducting a search of articles in the PubMed, Medline, Science Direct and Google Scholar databases, between the months of June and November of 2017 after the year 2000. Of the 315 studies initially identified, only 12 complied with the established selection criteria. The methodological variability of the different studies allows to observe how through the tapping tasks (repetitive movements of fingers), or through isometric contraction tasks, it is possible to determine the different parameters of fatigue that are evaluated in each study, being a very used to address that topic. Muscle fatigue has been predominantly studied when induced by isometric tasks, with a greater number of investigations using this type of methodology, since, although finger tapping is a reliable procedure to evaluate the underlying neurophysiological mechanisms of fatigue, It has been explored

BACKGROUND: Dementia is a disease that is constantly evolving in older people. Its diverse symptoms appear with varying degrees of severity affecting the daily life of those who suffer from it. The rate in which dementia progresses depends on different aspects of the treatment, chosen to try to control and slow down the development of the illness. OBJECTIVE: The aim of this study is to assess the effectiveness of cognitive training through a Brain Computer Interface (BCI) and the NeuronUp platform in two age groups whose MMSE is between 18-23 MCI (mild dementia). METHOD: 32 subjects took part in the study. There were 22 subjects in Group 1 (61-69 years of age) and 10 subjects in Group 2 (70-81 years of age). The criterium for the selection of the groups was to identify the age range with greater improvements due to the training. In order to estimate neuropsychological performance, the subjects were evaluated with the Luria-DNA neuropsychological battery before and after training. This design enables us to evaluate five cognitive areas: visuospatial, spoken language, memory, intellectual processes and attention. RESULTS: After training, Group 1 showed significant improvements in almost all the variables measured when compared with Group 2. This reveals a significant increase in cognitive ability, the degree of which depends on the age. CONCLUSION: People with mild dementia may delay cognitive impairment with a suitable cognitive training program.

People with severe physical disabilities may experience psychosocial problems. Boccia is one sport that athletes with severe disability can engage in, but no information on the effects of Boccia on psychosocial outcomes for participants with severe disability is available. Therefore, we analyzed the effects of Boccia on psychosocial outcomes in persons with severe disabilities. The study included two competitive Boccia groups: independent competitive (IC) (n = 9) and nonindependent competitive (NIC) (n = 7), as well as a recreational Boccia group (n = 14) and control subjects (n = 13) (mean age = 46.46 +/- 10.75). All participants underwent a rehabilitation program. Between-group differences in change scores were assessed using analysis of variance/multivariate analysis of variance. Within-group differences were compared using t-tests and effect sizes (ESs). Change in psychosocial parameters was not significantly influenced by study group (p > 0.05). All groups presented moderate-to-large ESs in physical and psychological quality of life (ES > 0.51). In comparison to the control group, who presented small-to-trivial ESs in General Health Questionnaire-28 (GHQ-28), State-Trait Anxiety Inventory, and Profile of Mood States-Tension, the IC and recreational group presented moderate ESs in GHQ-28, whereas the NIC group presented moderate ESs in anxiety and tension. In conclusion, the rehabilitation program had a general positive effect on the psychosocial status of individuals with severe physical disabilities. However, the competitive Boccia groups demonstrated a greater number of favorable changes, suggesting an added value of participation in Boccia.

The purpose of the current study was to establish measurement reliability in adults with Cerebral Palsy (CP), and to examine the feasibility and outcomes of an upper extremity strength training program (three times per week for 90 min each time). A control group design mixed with a prospective time series design for the intervention group was completed, including a pre-test, a post-test after a 12-week intervention period, and a follow-up in the intervention group after an additional 10-week period. Seventeen adults with CP with severe motor impairment took part in the study (10 in the intervention and seven in the control group). The test battery was comprised of wrist and hand dynamometry; dominant hand upper-extremity function measures (Jebsen Hand Function Test=JHFT, Minnesota Manual Dexterity Test=MMDT, and the Nine Hole Peg Test=NHPT); and activity of daily living with the Barthel Index. The results indicated that in both the control and the intervention groups, the strength tests exhibited good-to-excellent reliability during pre-test and post-test. The group comparison revealed that while in the pre-test no between-group differences existed, in the post-test the strength training group demonstrated significantly higher values in five out of eight strength measures, as well as in the MMDT. Discontinuing the program for eight weeks reversed the effects almost to baseline. In conclusion, the outcomes demonstrated the reliability of the assessments utilized in this study, as well as the feasibility of the strength training program, in adults with severe motor impairment due to CP.

En el ámbito educativo, uno de los temas más controvertidos es la inclusión de alumnos con discapacidad en los centros educativos ordinarios, tanto en primaria como en secundaria. Los mayores obstáculos y dificultades que señalan los docentes para la aplicación de la inclusión educativa alude a la falta de recursos materiales y personales, a la heterogeneidad de los alumnos en los grupos de clase y a la falta de preparación para la atención a la diversidad, provocando todo ello un amplio malestar entre el profesorado. La inclusión debemos plantearla desde un modelo ecológico (Hutzler, 2005) donde no sólo se basa en la instrucción si no en la tarea, en el entorno, en el patrón de movimiento y en el alumno para poder llevar a cabo la habilidad prevista (AU)


In the educational area, one of the most controversial topics is the inclusion of students with disability in the educational ordinary school, so much in primary as in secondary education. The main obstacles and difficulties indicated by the teachers are the lack of material and personal resources, related to the students heterogeneity in the different class group as well as the absence of preparation in the attention to the diversity. This inclusion must be raised from an ecological model (Hutzler, 2005) not only based on instruction, but in task, in environment, in movement and in the student to carry out the skill (AU)
